A new study from the International Council on Clean Transportation (ICCT) has linked road vehicle pollution to tens of thousands of premature deaths in the United States each year, highlighting the ongoing public health impact of emissions from cars, trucks, and buses.
The report claims an estimated 41,800 premature deaths in the U.S. during 2024 were associated with exposure to pollution generated by road transportation. That works out to roughly five deaths every hour, making vehicle emissions a significant contributor to air pollution-related health problems.
The researchers also found that road transport pollution contributes to a substantial number of childhood asthma cases, with the United States accounting for around one in every 10 new pediatric asthma cases linked to vehicle emissions worldwide.
The findings form part of a global analysis examining how road transport pollution could affect public health through 2050.
What The Study Found
The ICCT evaluated pollution produced throughout the fuel cycle for road vehicles and estimated the resulting health impacts using established epidemiological methods.
Globally, the organization estimates that road transport pollution contributed to nearly 700,000 premature deaths and around 250,000 new childhood asthma cases in 2024.
The study considered exposure to several major pollutants, including fine particulate matter (PM2.5), ground-level ozone (O₃), and nitrogen dioxide (NO₂), the latter being included for the first time in one of the ICCT’s global health assessments.
Researchers concluded that road transport pollution currently contributes to one premature death somewhere in the world approximately every 45 seconds.
Electrification Could Reduce Health Impacts

The report argues that a faster transition to zero-emission vehicles could dramatically reduce those health effects. Under an ambitious electrification scenario, the ICCT projects global premature deaths linked to road transport pollution would fall by roughly 63% by 2050 compared with 2024 levels.
The organization estimates that nearly 8.8 million premature deaths and around 1.7 million childhood asthma cases could be prevented worldwide between now and 2050.
For the United States specifically, the study estimates that reaching full market adoption of zero-emission passenger vehicles, trucks, and buses by 2040 could prevent more than 100,000 premature deaths and avoid more than 42,000 new childhood asthma cases by mid-century compared with current adoption trends.
Heavy-Duty Vehicles Play A Major Role

While heavy-duty vehicles represent less than five percent of the world’s vehicle fleet, the study found they produce a disproportionately large share of harmful emissions.
The ICCT says trucks and buses account for approximately 60% of tailpipe nitrogen oxide emissions, 55% of fine particulate emissions, and 65% of sulfur dioxide emissions from road transport globally.
Older vehicles lacking modern emissions controls were also identified as major contributors despite representing a relatively small share of total vehicle activity.
Study Highlights Policy Debate
The researchers argue that electrification alone is not enough to deliver immediate improvements. They also recommend stricter emissions standards, cleaner fuels, continued investment in charging infrastructure, and policies aimed at replacing older, higher-polluting vehicles.
As with any modeling study, the projections depend on the assumptions used for future vehicle adoption, emissions reductions, population growth, and exposure estimates. Even so, the report adds to a growing body of research linking transportation-related air pollution with significant public health impacts.
With governments around the world continuing to debate emissions regulations and the pace of EV adoption, the findings are likely to fuel further discussion about the long-term health benefits of reducing vehicle emissions.
